This form is for Community Partners (both individuals and organizations) who desire to hold an event where the resulting funds will be donated to UWF for the benefit of and to support the mission of UW-Madison, its affiliates or UW Health (including UW Carbone Cancer Center).

  • Thank you for being a community partner with UW–Madison and affiliates. We want your event to be a success. Please complete the following information, and your submission will go to WFAA for approval. If you have any questions, please contact our Gift and Fund Management Team.

  • Please note: The value is determined by establishing the Fair Market Value, not cost of the benefit received.
  • Note, that this in combination with submitting this form is your signature agreeing to the terms.

    I/We have read the UWF Third-Party Fundraising Events Policies and Procedures. I /We agree to adhere to those guidelines in planning and executing our event/activity. I/We understand that the guidelines are not comprehensive and that all decisions for the event/activity, including safety precautions, remain the responsibility of the event/activity sponsor. The University of Wisconsin Foundation does not accept or assume liability associated with the event/activity.
